Health at Home – Private Duty Home Care Services

If you’re like most seniors, you enjoy living independently, in the place you call home, doing the things that you love. But sometimes, help is needed with activities of daily living. Health at Home provides that help and allows you to choose how much or how often that help is provided. Whether you need a ride to the grocery store or a doctor’s appointment – or someone to prepare a light meal for you when you return – private-duty home care services provided by Health at Home are available for a few days or every day, 24/7, with a personalized plan of care created just for you.

Here are just a few of the many services private-duty home care provides:

  • Companion Home Care Services can include 24-hour care, if needed; could provide transportation, accompany you on outings, appointments or vacations
  • Home Management can include light housekeeping; laundry and linen changes; transportation; help with meal preparation; help with bathing, dressing and grooming
  • Memory Care can include wake-up and tuck-in service; home safety checks; respite care
  • Private Duty Care can include assistance with walking, transferring, therapies, checking and recording vital signs, medication reminders; help with household chores

Health at Home companion / homemaker services may be supervised by a Registered Nurse per state regulations.

Health at Home – Home Health Care Services

Health at Home’s quality, Medicare-certified home health care provides doctor-prescribed home health care services, including skilled nursing; physical, occupational and speech therapy; and medical social work. These can all be delivered to you in your home, for a full continuum of care.

Here are some of the times your physician may prescribe home health care services:

  • Following a stay in the hospital, recovering from an accident, surgery or serious illness.
  • When you have just been diagnosed with a chronic disease like heart failure, respiratory or kidney disease, diabetes or Alzheimer’s disease.
  • When you have complex medical treatments requiring the use of medical equipment and/or medication monitoring.
